The new procedure for processing, issuing, exchanging, transferring, withdrawing, returning to the state, invalidating and destroying the passport of a citizen of Ukraine for traveling abroad

The new procedure for processing, issuing, exchanging, transferring, withdrawing, returning to the state, invalidating and destroying the passport of a citizen of Ukraine for traveling abroad, was approved by the Resolution of the Cabinet of Ministers of Ukraine on May 7, 2014 No. 152 (as amended by the Resolution of the Cabinet of Ministers of Ukraine on November 16, 2016 No. 1001).

The passport for traveling abroad is issued:

1) no later than in 20 working days from the date of registration of the application form to receive it;

2) not later than seven working days from the date of registration of the application form for its urgent receipt;

3) within a period of up to three months - in case of registration of the application form in a foreign diplomatic establishment;

4) within up to three working days - in connection with the need for urgent travel abroad, related to the urgent need for treatment of a departing person, the departure of a person accompanying a seriously ill patient, or the death of a relative who lived abroad

5) within 10 working days - in case of leaving for a permanent residence abroad a child - a citizen of Ukraine.

Nevertheless, over the past 6 months, the above-mentioned deadlines are constantly violated, and the State Migration Service of Ukraine is not able to organize work on the timely registration of passports.

Thus, registration (including for lost or stolen goods), the exchange of passports for departure abroad by territorial bodies / territorial divisions of LCA is carried out through the Main Computing Center of the Unified State Demographic Register (hereinafter referred to as the Main Computing Center of the Register) in cooperation with the State Center for Personalization of State Documents enterprise "Polygraphic combine" Ukraine "on manufacturing of securities"

Registration (including for lost or stolen goods), exchange and issuance of a passport for traveling abroad are carried out:

1) a person who has reached the age of 16 years - on the basis of the application form submitted personally to him;

2) a person who has not reached the age of 16, a person recognized by the court as incapacitated or incompetent, on the basis of the application form of one of the parents (adoptive parents), guardians, custodians or other legal representatives (hereinafter - legal representatives) in the event of registration including replacement of lost or stolen goods), the exchange of passports for traveling abroad in a foreign diplomatic establishment - on the basis of a request form of one of the parents / legal representatives and the written consent of the second parent / legal (the application from the second parent / legal representatives is not required if he is a foreigner or a stateless person). In the absence of a second parent when submitting documents, such an application shall be submitted by a notary certified. If the parents are not married, the registration (including for lost or stolen goods), the exchange and issuance of a passport for traveling abroad is carried out on the basis of the application form of the one with whom the person lives;

3) a person who can not move independently in connection with a long-term health disorder and who needs urgent medical treatment abroad, which is confirmed by a medical report of the relevant health care institution, issued in accordance with the established procedure - on the basis of the application form of the person or his legal representative.

For registration (including for lost or stolen goods), the exchange of a passport for traveling abroad is made by an administrative fee, and for carrying out such actions abroad - a consular fee in the manner and in the amounts determined by law.

APPLICANT FOR THE PASSPORT DESIGN FOR EXIT FOR THE BORDER TO UKRAINE REPRESENTS THE FOLLOWING DOCUMENTS:

1) the passport of a citizen of Ukraine (for a person who has reached the age of 14 and residing in Ukraine)

2) a birth certificate or a document confirming the fact of birth issued by the competent authorities of a foreign state - in the case of issuing a passport for traveling abroad to a person under 14 years of age. If the parents or one of the parents of such a person were foreigners or stateless persons at the time of his birth, a certificate of registration of the person by a citizen of Ukraine is also submitted (in case the passport is first issued for traveling abroad with the use of the Register's funds);

3) a document certifying the identity of the legal representative and a document confirming the authority of the person as the legal representative - in the case of the filing of documents by the legal representative, unless the legal representative is one of the parents;

4) documents confirming the payment of the administrative fee, or the original document of exemption from its payment. The original of the document on exemption from payment of the administrative fee is returned to the applicant, and a copy of it is attached to the application form, certified by the employee of the territorial body / territorial division of LCA, the authorized entity by stating the "According to the original" and the signature indicating his position, surname, initials and date ;

5) in the case of issuing a passport for traveling abroad to a person who has not reached the age of 12 or a person, can not move independently due to a long-term health disorder and who needs urgent medical treatment abroad, as evidenced by the medical report of the relevant health care institution issued in the established order, - one color photograph with the size of 10-15 centimeters can be submitted for entering the digitized image of the person by scanning. The photo must comply with the requirements of the International Civil Aviation Organization (ICAO) Doc 9303.
