Submission of application: in person or through legal representatives.

Required documents:
    a statement indicating the applicant's details: surname, name and patronymic, passport data (series, passport number, date of issuance and issuing body, address), phone;
    billing document (invoice) with the bank, post office, or the code of the operation of the payment of the cost of the public service and the form of the passport;
    Ukrainian passport, but for a person under 16 years of age - a birth certificate (after the adoption of the documents returned to service recipients);
    Ukrainian passport for travel abroad;
    a copy of the registration number of the taxpayer registration card, in addition to individuals who, by their religious beliefs refuse to accept the registration number of the taxpayer's registration card and reported it to the relevant authority of the state tax service and have stamped in their passport;
    military ID or a document that substitutes it (for men from 18 to 25 years who have completed compulsory military service), or a certificate corresponding to the military commissariat of the possibility of departure from Ukraine (for men aged 18 to 25 years old to be called up for military service );
    for children aged from 14 to 16 years, their written consent notarized.

Duration of the application: Three months

Result: The issuance of the passport of citizen of Ukraine for traveling abroad to enter information about how to obtain permission to leave for permanent residence outside Ukraine

Reasons for refusal:

    Working with information constituting a state secret - before the deadline set by the Law of Ukraine "On State Secrets;
    are unsettled alimony, contractual or other obligations - to the implementation of the commitments or decisions dispute by agreement of the parties in cases stipulated by law, guarantee or liability, unless otherwise provided by international treaty of Ukraine;
    criminal case - until the end of the proceedings;
    conviction for committing a crime - until completion of sentence or release from punishment;
    evade the obligations imposed on him by a judicial decision, the decision of another body (official) - to the fulfillment of obligations;
    conscious alert false information about himself, - to determine the causes and consequences of the submission of false information;
    is subject to compulsory military service - to address the issue of deferment;
    a citizen filed a civil action in court - until the end of the proceedings;
    stay under the administrative supervision of the Interior, - before the termination of supervision.
